Clinical Pharmacist

Overview

As a Clinical Pharmacist at Interface Clinical Services, you will apply clinical expertise and commercial insight to support patients with chronic conditions in primary care. You will work with practices to provide clinical tools, identify opportunities for reviews, and ensure protocol-compliant service delivery while raising awareness of Interface within primary care. Responsibilities

• Support practices with clinical tools and resources for long-term condition management
• Deliver services within established protocols and operating instructions
• Identify opportunities for clinical reviews at practice, PCN, or ICB level
• Enhance awareness of Interface Clinical Services and build relationships with practices and local NHS organisations

Key requirements

• GPhC registered pharmacist
• minimum 2 years PQE
• primary care experience desirable
• PCPEP qualification desirable
• Full UK driving licence and willingness to travel; remote/home working allowed
