About the Role

This is a rewarding opportunity to join a private organisation delivering high-quality, personalised support to adults facing a range of social and emotional challenges – including mental health, isolation, life transitions, or safeguarding issues.

Support is provided through a flexible mix of face-to-face sessions , as well as remote contact via telephone and video link , allowing for responsive and accessible care tailored to each individual’s needs.

Key Responsibilities

Carry out person-centred assessments and develop individual support plans
Provide one-to-one emotional and therapeutic support
Deliver interventions in-person and remotely (telephone and video)
Monitor outcomes, manage risk, and follow safeguarding procedures
Liaise with families, carers, and external professionals to coordinate care
Maintain accurate and confidential case records
Contribute to ongoing service improvement

About You

Degree or equivalent qualification in Social Work
Registered with the appropriate UK regulatory body (e.g. Social Work England)
Strong interpersonal and communication skills
Counselling skills or therapeutic experience (desirable)
Confident working independently and remotely
