Our Adult Social Care service brings together experienced and supportive multidisciplinary teams who work collaboratively to make a real difference to the lives of adults across our community. We have a “can do” and “no wrong door” ethos. Our teams work together as one service and go the extra mile to ensure our residents have the right support.
We work closely with health and other partners to ensure people avoid unnecessary hospital admissions. If hospital care is needed, we make sure it is only for as long as necessary and we focus on helping them return home as quickly and safely as possible.
We have a range of intermediate care and discharge to assess services that focus on keeping people in their own homes.

We actively listen to social care staff about the best ways to deliver services and encourage feedback and suggestions for continuous improvement.
