Job Responsibilities:

• Conduct intake assessments and participate in case conferences with multidisciplinary teams.
• Collaborate with healthcare professionals to develop and review Individual Care Plans (ICPs).
• Monitor clients' progress and evaluate care outcomes.
• Conduct home and hospital visits when required.
• Provide guidance on relevant financial assistance schemes and community resources.
• Perform other duties as assigned by the reporting manager.

Requirements:

• Degree or Graduate Diploma in Social Work.
• Relevant experience in healthcare or community settings will be an advantage.
