• Conducting assessments and developing tailored support plans for young people leaving care.

• Collaborating with multi-agency partners to coordinate services and ensure comprehensive support for each individual.
• Facilitating life skills training and workshops to promote independence and self-sufficiency.

The ideal candidate will possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS, or equivalent).
• Registration with Social Work England.
• Experience working with young people, particularly those in or transitioning from care.
• Strong assessment and analytical skills, with the ability to develop effective support plans.
