Social worker-Assessment Team

As a Social Worker on the Assessment Team, your key responsibilities will include:

• Conducting comprehensive assessments of individuals and families to identify needs and risks.
• Developing and implementing tailored support plans in collaboration with service users and other professionals.
• Maintaining accurate and timely documentation in accordance with organizational policies and legal requirements.
• Providing ongoing support and intervention, ensuring the well-being and safety of vulnerable populations.
• Engaging with various stakeholders, including healthcare providers, community organizations, and law enforcement, to facilitate holistic support.

To be successful in this role, you must possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• Current registration with Social Work England (or equivalent regulatory body).
• Proven experience in assessment and intervention within a social work context.
• Strong communication, interpersonal, and problem‑solving skills.
• Ability to work effectively under pressure and manage a diverse caseload.
