Adult Social Worker needed for a Community Review Team in Brent, paying £32ph!

About the role:

We are recruiting an experienced Adults Social Worker to join the Community Review Team in Brent. This role focuses on reviewing care and support plans to ensure adults receive effective, person-centred services that promote independence and wellbeing.

Key Responsibilities:

• Complete Care Act reviews and reassessments
• Evaluate the effectiveness of care and support plans
• Work with adults with a range of needs, including older adults, physical disabilities and mental health
• Liaise with service users, carers, providers and partner agencies
• Identify risks, safeguarding concerns and required interventions
• Maintain accurate, timely case records
