At 360 CAMHS, we’re dedicated to making a meaningful difference to the lives of children and young people experiencing mental health challenges.

We specialise in crisis placements and complex transitions , providing tailored, trauma-informed care and support through our nationwide network of risk-assessed properties. Our focus is on creating safe, structured and nurturing environments where young people can feel secure, build trusting relationships and work towards positive, long-term outcomes.

We’re looking for a Care Coordinator to join our team and play a key role in coordinating high-quality care and support for the young people we work with.

The role

As a Care Coordinator, you’ll be responsible for coordinating care packages, supporting our care teams and ensuring that the young people we support receive consistent, person-centred and high-quality care.

You’ll work closely with young people, their families, care workers, Registered Managers, healthcare professionals and local authority stakeholders , helping to ensure care plans remain effective, responsive and focused on positive outcomes.

This is a varied role combining care coordination, crisis management, stakeholder communication, quality assurance, compliance and staff support . You’ll need to be organised, responsive and confident working in situations where priorities can change quickly.

What you'll be doing

Care coordination & crisis management

  • Coordinate care packages and support the allocation of care workers according to individual needs.
  • Manage emergency staffing requirements, placement escalations and immediate responses.
  • Maintain accurate care schedules and ensure care workers have access to up-to-date information and care plans.
  • Implement contingency arrangements when unexpected absences or emergencies occur.
  • Work with young people, families and healthcare professionals to develop person-centred and adaptable care plans.
  • Regularly review care plans alongside Registered Managers, ensuring they reflect changing needs, preferences and goals.
  • Ensure care plans contain all relevant information, including medication, dietary, mobility and emotional support requirements.
  • Maintain a strong focus on service delivery, quality and positive outcomes.

Working with young people, families and stakeholders

  • Build open, empathetic and trusting relationships with young people and their families.
  • Act as a key point of contact for stakeholders, responding to concerns, questions and requests relating to care.
  • Provide ongoing support and reassurance to young people and their families.
  • Attend risk management, safeguarding and other meetings relating to the care of young people.
  • Confidently represent the care and support plans of the young people you support in internal and external meetings, alongside the Registered Manager.
  • Develop positive, nurturing relationships with funding authorities and other stakeholders.
  • Monitor commissioned versus delivered hours and maintain accurate records relating to staffing costs and changes.

Compliance & safeguarding

  • Maintain awareness of relevant CQC regulations, standards and guidelines.
  • Ensure care delivery and documentation meet required regulatory standards.
  • Maintain accurate records of care activities, assessments and changes in the needs or circumstances of the young people we support.
  • Support the management team with CQC inspections, audits and compliance activity.
  • Monitor key performance indicators relating to compliance, contact, care quality and service delivery.
  • Identify, document and promptly escalate safeguarding concerns, incidents and emerging risks.

Supporting our care teams

  • Provide ongoing guidance, support and encouragement to care workers.
  • Monitor staff availability and anticipate potential staffing shortages.
  • Help minimise avoidable cancellations and maintain consistent staffing teams to promote continuity of care.
  • Ensure staff complete mandatory and relevant additional training.
  • Support the recruitment, onboarding and orientation of new care staff.
  • Help create a culture built on trust, honesty, openness and continuous learning.
  • Promote a positive, no-blame culture where safeguarding concerns and whistleblowing can be raised safely.

Quality assurance

  • Support robust quality assurance processes, including client feedback and audits.
  • Review feedback and help address concerns or identify opportunities for improvement.
  • Work with the management team to implement improvements and corrective actions.
  • Support the Registered Manager with the implementation of service priorities and improvements.
  • Monitor the satisfaction of young people and their families, always looking for ways to improve their care experience.

What we’re looking for

We’re looking for someone who is compassionate, organised and proactive , with the confidence to coordinate care in a complex and sometimes fast-moving environment.

You’ll need to be able to build strong relationships with young people, families and professionals, while also maintaining a clear focus on quality, compliance and safe care delivery.

You should be comfortable managing competing priorities, responding to unexpected situations and communicating confidently with a range of stakeholders.

What we offer

We’re committed to supporting our people to develop their skills and knowledge.

As part of your induction and development, you’ll receive:

  • A comprehensive induction.
  • Fully funded Care Certificate training.
  • Safeguarding training.
  • Clinical training relevant to the role.
  • Ongoing support and development.

Important information

This is a role with direct access to young people , so a full Enhanced DBS check will be required.

Due to the nature of CAMHS services, participation in an out-of-hours on-call rota is a requirement of the role. There may also be occasions where flexibility outside standard working hours is required to respond to emergency staffing needs or placement escalations.

Join 360 CAMHS

If you’re passionate about making a genuine difference to the lives of children and young people and have the organisation, compassion and resilience to coordinate high-quality care in a complex environment, we’d love to hear from you.
