Somerset Council is seeking an experienced Social Worker in the Learning Disabilities team to support adults with learning disabilities to live independently, with choice and control. You will assess needs, manage risk and develop personalised care plans in line with Care Act 2014.
You will build long-term relationships with individuals and carers, work with partner agencies, and ensure safeguarding and professional standards are upheld in a supportive, development-focused environment.
