About the Role
You will work within Adult Social Care services, supporting adults using a strengths-based and person-centred approach to maximise independence, wellbeing, and choice.
Key responsibilities include:
Completing complex Care Act assessments and reviews.
Undertaking risk assessments and care planning.
Contributing to Continuing Healthcare (CHC) processes.
Applying the Mental Capacity Act, including capacity assessments and Best Interest decisions.
Identifying enablement and reablement opportunities.
Leading safeguarding enquiries, including complex safeguarding investigations.
Working closely with health professionals, voluntary organisations, and multi-disciplinary teams.
Undertaking Court of Protection work, including Deprivation of Liberty applications within the community where required.
Maintaining accurate and timely case records.
Supporting service development and continuous improvement initiatives.
Requirements
Social Work England registration.
Experience within Adult Social Care.
Strong knowledge of the Care Act, Mental Capacity Act, Mental Health Act, and safeguarding legislation.
Experience completing assessments, reviews, safeguarding enquiries, and support planning.
Ability to manage a varied caseload and work independently.
Excellent communication and partnership-working skills.
Full UK driving licence and access to a vehicle.
This role would be particularly suited to Social Workers with experience in Adult Community Teams, Safeguarding Teams, Locality Teams, Physical Disabilities, Older Persons, or Learning Disabilities services.
