Acute Mental Health Liaison Clinician

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Mental Health Practitioner to join the Mental Health Liaison Team within a general hospital. The post involves working in a 24/7 service, including weekends and out-of-hours, to assess and manage mental health needs alongside acute physical health colleagues.

You will conduct holistic assessments, manage risks, and develop recovery-focused care plans with patients and carers as part of a multidisciplinary team.
