Care Co-ordinator, Mental Health (Community) | Flexible Hours & CPD

Pennine Care NHS Foundation Trust is recruiting a Care Co-ordinator to assess, treat and deliver care to adults with complex mental health needs within the Tameside West CMHT. You will ensure high-quality care aligned with Trust policies and regulatory standards.

You will undertake holistic assessments, manage a varied caseload and contribute to clinical decision making within a multidisciplinary team, supporting patient enablement and recovery in the community.
