Frailty ACP: Advanced Nurse Practitioner with Home Visits

The Grove Surgery in Christchurch is seeking a Frailty Nurse Advanced Clinical Practitioner (ACP) to join the South Coast Medical Group PCN on a full-time basis. Based at The Grove Surgery, you will work across the PCN delivering proactive, high-quality frailty care to a predominantly housebound patient population.

This autonomous role combines frailty expertise with advanced assessment, diagnosis, prescribing and clinical decision-making to support improved patient outcomes.
