Social Prescribing Co-ordinator

Overview

In this role you will connect prisoners to activities and services, supporting their health and wellbeing while reducing dependency on medications. You will assess referrals, craft personalised wellbeing plans, and manage a caseload to help patients access prescribed services confidently. You’ll coordinate with prison and health teams to deliver a whole-system, asset-based social prescribing approach. This position offers regional development opportunities and practical on-site work across multiple sites with a clear impact on lives.

Responsibilities

• Complete initial assessments for referrals to the social prescribing service
• Assess patient wellbeing and identify what matters to them
• Develop personalised wellbeing plans with practical, social, and emotional support
• Manage a caseload with follow-up to ensure access to services
• Schedule and sequence social prescribing activities
• Liaise with Health and Prison departments to raise awareness and support delivery
• Foster cross-departmental and regional collaboration to meet system-wide aims

Key requirements

• Experience as a social prescriber (desirable)
• Experience using SystmOne (desirable)
• Experience providing information, advice and guidance in a health/care setting
• Experience working in a prison environment or with patients with complex needs
• Experience managing own caseload and prioritising patient needs

• Positive, inclusive and compassionate practitioner
• Strong communication and interpersonal skills
• Ability to work in challenging environments and collaborate across teams
• Experience using SystmOne
