In-Home Rehabilitation Support Worker (Part-Time)

East Sussex Healthcare NHS Trust is seeking a part-time Health Care Support Worker to join an integrated Home Care/Joint Care Reablement (JCR) service in Lewes. You will work in clients’ homes, provide rehabilitation, and support earlier discharges from hospital within a seven-day service.

CPD and supervision are embedded; travel across the county is required. DBS check is mandatory; enhanced disclosure applies.
