Community Mental Health Nurse – Older Adults (Band 6)

Pennine Care NHS Foundation Trust in Stockport invites applications for a Band 6 Community Mental Health Practitioner in Older Adult Services. You will work in the community with adults with severe and enduring mental health issues, delivering person-centred care.

You will assess patients, manage a complex caseload, and liaise with in-patient areas and MDT colleagues. The role involves home visits and contributing to crisis duty within a supportive, multidisciplinary team.
