Our Rapid Response service provides urgent assessment and intervention, while our Virtual Ward enables patients with complex needs to receive intensive specialist support without hospital admission. Working alongside a highly skilled consultant-led, multidisciplinary team, you'll use your clinical expertise, decision-making skills and compassionate approach to support patients and families when they need us most., We're looking for People who Are passionate about community-based care.
