Cambridgeshire County Council is recruiting three Adult Support Coordinators for the Home Care Project Team in Huntingdon. The role focuses on strengths-based reviews and delivering a new Home Care Framework across Cambridgeshire to improve outcomes for adults with learning disabilities, physical disabilities and older people.
You will work with individuals, families and carers, ensuring care plans reflect current needs and promote independence, wellbeing and positive outcomes.
