Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Structured Clinical Management Practitioner for the Complex Needs Service based in Macclesfield with a cross-site travel within East-Cheshire.
You will manage a capped caseload and deliver SCM interventions across assessment, stabilization and intervention phases, work collaboratively with clients to develop a shared understanding, attend SCM supervision, and promote evidence-based practices across care teams.
