Home-First Early Intervention Occupational Therapist

Birmingham Community Healthcare NHS Foundation Trust is recruiting for an Early Intervention Community Team role focused on home-first rehabilitation, delivering high-quality care in patients' homes and community settings.

You will work multi-professionally within an ambitious MDT, leading and promoting a culture that prevents avoidable hospital admissions and supports early discharge, helping people access the right care at the right time.
