Imperial College Healthcare NHS is seeking an experienced professional to join the Discharge team at Charing Cross Hospital. You will manage complex discharge pathways, coordinate multi-disciplinary care, and ensure safe, timely transfers of patients with complex needs.
You will collaborate with ward teams, participate in staff development, and support weekend and holiday rota commitments as part of a dedicated discharge team.
