Cambridge University Hospitals NHS Foundation Trust is seeking a Band 4 Discharge Co-ordinator to join the Early Intervention Team. You will work within Emergency Department, SDEC and short stay wards to coordinate adult patient discharges and support timely, safe planning.
You will collaborate with Discharge Planning Specialist Nurses and Frailty Nurses, tracking progress and escalating issues as part of a multidisciplinary team in a busy hospital setting.
