Community Nursing Assistant - Home Care & Patient Impact

Gloucestershire Health and Care NHS Foundation Trust is seeking a Community Nurse to join our Integrated Community Team, delivering nursing care to patients in their homes across the Gloucester area.

You will undertake clinical tasks such as monitoring, wound care, medications administration, and records updating, working within a multidisciplinary team to support timely, patient-centred care and prevent unnecessary hospital admissions.
