The Clinical Nurse Specialist role is based within Woodlands Hospice Ambulatory Specialist Palliative Care Team and supports a CNS-led model of care for palliative patients in the community.
As a Band 6 CNS, you will manage a defined caseload, participate in nurse-led clinics, and coordinate care across hospital, hospice and community settings, including on-call and weekend coverage where needed.
