Waythrough is seeking a GP Hub Recovery Coordinator for the HAGA service in London. You will join a multi-disciplinary team supporting people affected by substance use, engaging them early and guiding them to appropriate treatment, recovery and community services.
You will work with GP practices, healthcare professionals and partner agencies to provide joined-up support, delivering personalised plans, facilitating peer support and helping to develop sustainable community networks.
