CARE COORDINATOR – PERMANENT OPPORTUNITY

Location: GL2 4NE

Salary: £26,000 per annum

Hours: 37.5 hours per week

Monday–Friday | Full-time

We are currently recruiting for an experienced Care Coordinator to join Eden Housing & Support.

The Role

You will play an important role in coordinating and supporting the delivery of high-quality care services. The role will involve working closely with care staff, clients and other professionals to ensure services are well organised and effectively delivered.

Key responsibilities include:

• Coordinating care and support services
• Managing staff rotas and shift cover
• Supporting care staff and dealing with day-to-day queries
• Maintaining accurate care plans, records and documentation
• Completing and reviewing risk assessments
• Liaising with clients, families and professionals
• Supporting compliance and quality standards
• Responding to emergencies when required

About You

We are looking for someone with previous Care Coordinator experience or strong transferable experience in a Field Care Supervisor, Care Supervisor, Senior Carer or similar role.

• Previous care coordination/supervisory experience
• Strong communication and organisational skills
• Experience with care plans and risk assessments
• Ability to coordinate staff and respond to changing situations
• Confident working with clients, families and care professionals

No specific qualifications are required, although relevant care qualifications and experience would be beneficial.
