You will assess and support adults with complex needs, working within a multidisciplinary team across Wiltshire, with hybrid working and flexible base locations such as Marlborough, Chippenham, Warminster or Salisbury.
The role offers NHS Band 6 AfC terms, with the opportunity for professional development and supervision, and a strong focus on improving health and independence
