End-of-Life Home Care Assistant – Flexible 30h

Cornwall Partnership NHS Foundation Trust in St Austell is offering two Community Healthcare Assistant posts at Band 3 on a permanent contract. Each post is 30 hours per week, covering weekends and evenings on a rota, with mileage paid when travelling to patients’ homes.

The role provides end-of-life and at-home care as part of the Continuing Healthcare at Home service, requiring compassionate, patient-centred care and the ability to work alone or with nursing colleagues.
