On-site NHS Social Prescribing Coordinator (Full Time)

NHS Jobs in Reading is inviting applications for a Social Prescribing Co-ordinator to join our healthcare teams. This is a full-time role and will require on-site work across our sites as part of the position.

You will support patients by linking them to community services, coordinating referrals, and ensuring smooth pathways into social care and wellbeing activities. Hours are typically Monday to Friday, 8am–4pm or 9am–5pm.
