Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Band 6 Community Diabetes Specialist Nurse to join a nurse-led team. You will provide specialist assessment, education and support to adults with diabetes across community and primary care settings.
You will lead on initiating and optimising glucose-lowering therapies, including insulin, and deliver education to patients and professionals while contributing to service improvement across the locality.
