Shared Lives Care Practitioner — Home-Based Support & Innovation

Sheffield Shared Lives is recruiting two Adult Social Care Practitioners to coordinate and manage a diverse caseload and supervise Shared Lives carers. The role involves building relationships with carers, people who are supported, families and other professionals, and promoting innovative, high-quality services.

We offer flexible home working with office-based team meetings and regular home visits in the community.
