Senior District Nurse: Lead Community Care at Home

West London Mental Health Trust is seeking a Band 6 Senior Community Nurse/District Nurse to join the Greenford locality. You will lead a community nursing team, drive admission avoidance, promote health at home, and deliver holistic, patient-centred care within integrated services.
