Description:

We have an exciting opportunity for an CPN to work with an NHS client of ours in Essex, Canvey Island. This full-time locum position is based in Essex, Canvey Island; the service operate Monday to Friday 0900 til 1700. You must be available for the full post.

The role is set to begin ASAP and will last for 4 weeks initially with a position to extend. This role is offering an hourly rate of £27 Per Hour. Please note that this position is inside IR35.

To be suitable for this role, this role requires you to to be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. To provide professional advice and support to other agencies and members of the multidisciplinary team, to act as a role model and resource for junior/less experienced staff.
