Temporary Care Coordinator, TeleHealth

• Conduct patient outreach to support enrolment into the Asthma Virtual Care programme.
• Monitor the patient dashboard and review chatbot responses, routing and escalating cases to the clinical team according to established workflows.
• Follow up with non-responsive patients and guide them on using the chatbot where required.
• Verify patient eligibility and ensure enrolment details are accurately reflected in the system.
• Coordinate appointments, referrals, handovers and communication between patients and clinical teams.
• Maintain accurate and timely patient records, interaction notes and escalation details across relevant systems.
• Collate and validate programme data including enrolment, response rates, outreach attempts, escalations and patient outcomes.
• Analyse trends and care gaps to identify opportunities to improve patient engagement and operational performance.
• Prepare regular reports and support audits, service evaluations and continuous improvement initiatives.

Requirement

• Diploma in Health Services Management / Nursing / Community Care / Social Services
