East of England Community Health & Care NHS Trust is seeking clinicians to work in a multidisciplinary Crisis Team, assessing referrals from GPs, CMHTs and ward teams, and delivering home treatment to patients on our caseload.
You will provide gatekeeping and feedback based on CRHTT criteria, undertake comprehensive risk assessments, support hospital wards, facilitate early discharge, and promote recovery and safe care in the community.
