Compassionate Palliative Home Care Assistant

Trinity Hospice is seeking a Health Care Assistant to join our Community Nursing Team, supporting patients and families in their own homes. You will provide practical, personal and emotional care, working with specialist nurses to ensure comfort and dignity at the end of life.

We value compassionate, patient-focused care, strong communication, and the ability to work both independently and within a multidisciplinary team.
