Community Diabetes Care Assistant - Part-Time (30h/wk)

Surrey Downs Health and Care Diabetes Team in Leatherhead is recruiting a Band 3 Community Diabetes Health Care Assistant to join our community team across Surrey Downs. The post supports the Diabetes Specialist Team, delivering patient education and diabetes care, with a 30-hour per week contract.

You will assist with health screening, glucose monitoring, and education, with a 5-day week 8:30am-4:30pm, and opportunities for development.
