Reablement Assessor - Empower Daily Living & Independence

Portsmouth City Council is recruiting for a role based in the office with community visits, working on a rota covering Monday to Sunday. The post involves initial assessments, care planning, and ensuring smooth transitions from hospital or community settings while promoting independence.

You will support service users with daily living tasks, maintain records, and supervise Reablement Support Assistants. A driving licence and ability to travel within the community are required.
