You will support discharge planning, liaise with MDT, patients and carers, and ensure the patient journey ends with a safe, timely discharge.
The role requires excellent communication, a caring approach, Enhanced DBS clearance for children and adults, and NVQ Level 3 or a Diploma in Healthcare.
