Care Co-ordinator Community Mental Health Flexible

Pennine Care NHS Foundation Trust is seeking a Community Mental Health Practitioner for the Tameside West CMHT to support working age adults in Dukinfield and surrounding areas.

You will undertake care coordination, holistic mental health assessments, and evidence-based interventions. You will manage a caseload, participate in MDT meetings, supervise student nurses, and liaise with social care partners while ensuring compliance with policy and safeguarding requirements.
